You have the right to complain to us and to the federal government if you believe your privacy rights have been violated. You have a right to a paper copy of the entire Notice of Privacy Practices.
We reserve the right to make changes to the Notice of Privacy Practices. A copy of the current Notice is available in the locations where you receive services.
